Telehealth is the use of electronic information and telecommunications technologies to support health care, including health education and population health management
Telemedicine
Telemedicine is a subset of telehealth in which a patient's health status is improved in a remote clinical service through the use of medical information exchanged from one site/provider to another via electronic communications
Ixora Primary Care™ Services
For emergencies (i.e. chest pain, shortness of breath, decreased arousal/responsiveness, change in mental status, "worst headache of life", numbness/tingling in extremities especially if one sided, etc), please call 911 or go to the emergency department.
General Services offered include:
1. Adult chronic disease management (eg. hypertension, diabetes, cholesterol, COPD etc)
2. Adult acute illness management (eg. URI, sore throat, COVID, influenza, pneumonia, UTI, etc)
3. Acute pediatric illness management (eg. URI, fever, ear ache, rash, etc)
4. ADHD management
5. Weight loss assistance
6. Form/service requests (eg. work excuse, school excuse, medication refills, medical exemption letters as is appropriate, etc)

Direct Primary Care Membership
-Age 0-17: $80/month
-Age 18-64: $100/month
-Age 65+: $120/month
**One time registration fee of $50. There will be a $10 discount per household member that registers at the same time.
Membership Benefits
-30 minute appointments for adults (20 mins for acute pediatrics)
-up to 5 appointments/visits per month
-unlimited communication and access to your provider
-priority scheduling with WEEKEND, SAME DAY, and AFTER HOURS appointments (upon request)
-free requests/paperwork/refills/letters
-$50 home visits for members within Palm Coast, Flagler Beach, Daytona and St Augustine, Florida areas. **Home visits are not currently available outside these areas.

PAYMENT IS EXPECTED AT THE TIME OF SERVICE. Payment methods include major credit cards, debit cards, HSA accounts. Please note that there is a $25 no-show fee for appointments that are not cancelled within 24 hours notice.
All patient information is private and will not be shared with any 3rd party outside pertinent providers/referrals/labs/imaging
