Prices, payment and insurance information

In order to provide appropriate, comprehensive and quality care for our patients, Ixora Primary Care™ does not accept insurance and functions as a direct-pay practice.  This allows the provider to provide care/service without limitations of time, cost, prior authorization approvals, overscheduling, all of which have been shown to delay and impede access to care.

 

Services will be considered "out-of-network".  Receipts can be provided for submission to your insurance company for possible out of network reimbursement.

Direct Primary Care Membership

-Age 0-17:  $80/month

-Age 18-64: $100/month

-Age 65+:  $120/month

**One time registration fee of $50.  There will be a $10 discount per household member that registers at the same time.

 

Membership Benefits

-30 minute appointments for adults (20 mins for acute pediatrics)

-up to 5 appointments/visits per month

-unlimited communication and access to your provider

-priority scheduling with WEEKEND, SAME DAY, and AFTER HOURS appointments (upon request)

-free requests/paperwork/refills/letters

-$50 home visits for members within Palm Coast, Flagler Beach, Daytona and St Augustine, Florida areas.  **Home visits are not currently available outside these areas.

PAYMENT IS EXPECTED AT THE TIME OF SERVICE.  Payment methods include major credit cards, debit cards, HSA accounts.  Please note that there is a $25 no-show fee for appointments that are not cancelled within 24 hours notice.
